The Host Home Coordinator will support youth and young adults with skill building, independent living skills / coaching, resource referrals and other supports based on individual needs as well as providing coordination, support and training for Hosts.
Services are primarily provided in the community.
Positive outcomes are achieved through purposeful and respectful relationships rooted in trauma informed care, hope cultural humility and growth mindset.
An openness to embrace and explore bias, equity issues, successes and challenges from a place of understanding and curiosity is needed to succeed and develop in the position /organization.
