The Case Manager/Clinician is a key associate at Hope Network whose primary responsibilities include:
Coordinate all services necessary to maximize child's success in treatment and growth in programIn residential settings, this position provides clinical and program consultation to direct care staffTrauma informed approach is necessaryEnsure case management compliance with agency standards and the practice model.

1Regular and predictable attendance is an essential requirement of this position.
Clinical
Responsibilities:
1Provide comprehensive psychosocial assessments.
2Perform individual and group therapy that is suited to the needs of each childA minimum of 0.
5hrs of individual therapy and 2hrs of group therapy are required.
3Meet with all kids on case management case load at least weeklyA minimum of 0.
5hrs of case management contact is required.
4Coordinate and ensure child involvement in the Person-Centered Planning Process (PCP) for assigned consumersAssure responsibility for the development & implementation of each assigned child's Individual Plan of Service.
5Link children to necessary ancillary services.
6Act as an advocate and/or a liaison coordinating services among providers of service and referral source.
7Maintain children's records - meeting all appropriate standards according to HNBHS and all regulatory bodies.
8Participate in legal proceedings including petitioning and testifying in clinical emergencies.
9Facilitate the coordination of children's intakes, discharges, and transfers to and from other facilities programs, agencies.
10Ensure that children are transported to appointments, accompanying them when medically necessaryObtain relevant medical/treatment information from appointments.
11Link and coordinate follow-up of professional recommendations.
12Facilitate communication with guardians, payees, and family members with regard to children's care, financial and treatment issues.
13Meet with assigned children face-to-face as medically necessary in their Individual Plan of Service.
14Provide after hours on-call services as designated by HNBHS.
15Facilitate hospitalizations as needed and monitor progress by attending hospital medical/team meetings and coordinating with hospital social worker.
16Provides consultation to direct care staff regarding clinical and program issue, where applicable.
17Participate in children's program reviews and team meetings, where applicable.
18Provide coverage for case-managers on PTO as needed.
Administrative
Responsibilities:
1Participate in committees, task forces, and other related meetings and activities as assigned by supervisor.
2Practice according to licensing and accreditation for assigned program(s).
3Attend agency meetings:
clinical, administration and management.
4Network with relevant agencies and providers as needed.
5Follow general safety practicesReport any unsafe conditions to the manager of that location.
6Complete and submit Service Activity Logs (SAL's) in accordance with agency policy/protocols.
7Coordinate benefits and verify benefit status monthly.
8Other duties as assigned by supervisor.
Educational / Talent Requirements:
1Minimum Education required:
Master Social Work Degree, Master Counseling Degree, or Master Psychology degree and eligible for licensure.
2Valid driver's license with acceptable driving record according to Hope Network insurance carrier definitions.
3Must complete a Central Registry Clearance form for the State of Michigan and any state in which residence and/or employment was previously establishedMichigan Central Registry Clearance must be completed on an annual basis.
4History of working with children with intensive therapeutic and behavioral needs is highly preferred.
5A comprehensive knowledge of Trauma-Informed Care and Gentle Teaching foundational principles and experience implementing and working within these frameworks is preferred.
6Experience, comfort, and proven effectiveness leading trainings and treatment-based discussions amongst a variety of staff, including clinical, administrative, and direct care, is preferred.
7Experience and knowledge working within the Michigan Children's Foster Care system as well as the Michigan Department of Health and Human Services is preferred.
Work Experience Requirements:
1Prior case management experience in a mental health or rehabilitation setting preferred.
2Two years working with children in the foster care, mental health, or residential care, preferred.
3Ability to communicate effectively in both verbal and written formats, to meet position requirements.
4Ability to articulate and actively support the Hope Network mission to various audiences.
5Ability to work autonomously and as a team.
